Our main service area is in Gage, Jefferson, Pawnee, and Saline counties. Our team all lives in the counties we serve which allows us to maintain prompt response time to patient needs. Our promise is to respond to inquiries for hospice enrollment within an hour of receiving notification. Once we have clarified eligibility and spoken with the patient and caregivers we look to set up resources and care within that first 24hrs. As soon as it becomes a thought!  Hospice currently is the most underutilized Medicare service available. It is never too early to have a discussion about what hospice looks like and our team is happy to help clarify goals, services available, and perhaps alleviate misconceptions with your patients. We recognize these are tough conversations and take time. Allow us to alleviate some of that burden and provide the time and answers necessary to help make a hard decision a little bit easier. 

All patients are given the choice when they enroll to maintain their PCP or transition their PCP to the Hospice Medical Director. We recognize how important the provider-patient relationship can be, and certainly want to honor that. We also want to serve as a resource and are ready to step in fully when needed.
